Breast MRI can be a useful surgical planning tool for difficult breast cancer cases. A study out this week in the Journal of Surgical Oncology describes a novel automatic segmentation process that reconstructs the nipple areola complex (NAC) within breast MRIs and calculates the three-dimensional (3-D) distance between the NAC and tumor. A distance of 2.1 cm was the optimal cutoff for ruling out NAC involvement with a negative predictive value of 89%. This adds to diagnostic mammography when preparing for a nipple-sparing surgical technique in providing surgeons with detailed 3-D information about the proximity of tumor to the NAC and risk of involvement. It’s great if you have a special kNACk for clinically ruling out NAC involvement with breast exams alone, but this isn’t bad additional info to obtain before cutting on a tumor that’s cutting it close.
