In this randomized phase 2 trial, patients receiving radiation for glioblastoma were randomized to proton therapy or IMRT to assess any differences in neurocognitive function. Cognitive measures included the HVLT-R, TMT, and COWA. Though protons resulted in significantly lower dose to most critical structures, there was no significant difference in the time to any cognitive failure between arms. Patients in the IMRT arm experienced more cumulative grade 2+ toxicity at an average of 1.15 events versus 0.35 for protons. However, patient reported outcomes were no different at 6 months, and there were no differences in progression-free and overall survival. | Brown, Neuro Oncol 2021
