Recalibrate.

This SEER analysis demonstrates, among women with node-negative ER(+) breast cancer (n=51,336), those categorized as non-Hispanic Black are at higher risk for cancer mortality across all Oncotype risk groups when compared to their White counterparts. What’s more, the prognostic accuracy of Oncotype was significantly lower for Black women—not surprising given they made up a small minority of the cohorts used in development and validation. Good news is that Black women with recurrence scores <25 still carried only a 1% risk of death with endocrine therapy alone, suggesting no need for treatment escalation in this population. TBL: While specific biologic and/or sociologic influences remain elusive, Black women are at higher risk for breast cancer death across Oncotype risk levels. | Hoskins, JAMA Oncol 2021
