Lack of standard involvement of supportive care in the management of patients with life-limiting cancer is nearly ubiquitous. Oncology fellows at Hopkins pinpointed an easily-remedied workflow issue that was hindering ease of referral to their supportive care colleagues: difficulty locating the appropriate order and confirming an appointment through the electronic medical record system. After standardizing the referral process with a single order set and a single scheduler who would communicate with both the referring and accepting physicians, rates of supportive care use for patients with advanced cancer soared from 12% to nearly 48%.
