Beside British witticisms and hyperbole, this editorial offers convincing arguments for abandoning the long-standard transrectal (TR) approach to prostate biopsies in favor of the newer perineal approach—likewise performed under local anesthesia in the office. While neither sound particularly appealing, the perineal approach goes straight to the money without transecting rectal flora, slashing rates of post-biopsy sepsis.
