Earlier this year, the Congress of Neurological Surgeons (CNS) published a stack of guidelines on the management of brain mets spanning surgery to radiation to chemo to supportive care. They sought ASCO and SNO oncologic input ex post facto and received ringing endorsements of the two most vanilla of their eight chapters, the ones suggesting steroids and anti-epileptics should be used as sparingly as possible and never as prophylaxis.
