In this trial, nodes from radical prostatectomies underwent molecular evaluation using quantitative polymerase chain reaction (PCR) for expression of Kallikrein 3 (aka KLK3, aka PSA). PCR accurately identified nodal mets in all 28 (25%) patients with pathology involved nodes. More interestingly, PCR detected molecularly involved nodes in another 32 (29%) patients with pathologically negative nodes. Who cares? Well, you should, because molecular nodal status was a better predictor of biochemical recurrence than pathologic nodal status.
